Update the statistics on Exercise => Report by question

On the exercise statistics there is an icon to show "Report by question"

It opens the page /main/exercise/stats.php that shows the list of questions and for each question a number of columns with statistics. One column is called "Number of learners who selected it" but it should be renamed with "Number of times the question was answered" and the statistics in this column should show the number of attempt for each question. (Change 1 in image below)

Below for each question it shows the list of possible answer and for each answer we have the column "Number of learners who selected it". This column should be modified with the title "Number of times this answer was selected" and the statistics in this a column should be adapted to show the corresponding number and not the number of user that selected it. (Change 2 in image below)
