In Contact > Phone, is_fax should apply to EACH number not ALL numbers.

Current behavior

• There's a checkbox for whether all numbers entered are fax numbers or not.

Expected behavior

• Each number entered should have a checkbox as to whether it is a fax number or not.

Steps to replicate behavior

1. Add a browse page in Wagtail.
2. Add an expandable.
3. Add a Phone.
4. Add additional phone numbers and see that there's only one checkbox to say whether they ALL are a fax or not.
