Upgrading from v0.10 to v0.11 - missing cainjector annotation

The annotation is missing from the table: old: certmanager.k8s.io/inject-ca-from new: cert-manager.io/inject-apiserver-ca

I'll try to get a few moments to submit a PR tomorrow!
