Add in-app printing functionality

Currently, the ASCVD app can be printed from any browser, but the view may not be consistent across all browsers. The app view may also be printed from within an EHR system if the system provides a way for the view to be printed. In addition, if a user wants to print all views of the app, they have to print each view separately.

It would be good if the app had a button on the header that allowed for making a print-friendly view of all views in one print preview (only if a score has already been computed). This would make it easier for users to print all parts of the app easily and allow for a consistent view to print across all mediums.
