Disabling an app removes it from the app list

Some MDMs disable and re-enable apps at will. This is treated as a 're-install' by RethinkDNS. And when users have Block newly installed apps by default enabled, these re-enabled apps are blocked too, 'causing (minor) annoyance of having to go through the unblock ceremony.

We can do better and handle disabled apps as disabled instead of uninstalled.
