Update Radiology Example

AUC Criteria include a "categorization", which is effectively the interpretation of the score. For the radiology use case, this corresponds to the display text for the score. Update the example to provide both the decimal score and the text representation

It could be really helpful to also include a flowchart or diagram (or quick start) specific to the GAO use-case due the potential number of developers unfamiliar with CDS Hooks who may consider it specifically for Radiology GAO.
