Custom dimension for blank document

Following #180

Add an option "Custom..." (FR: "Personnalisée ...") in dimension combobox.

Display 2 integer field (for unit millimeter), if possible with stepper, to define custom width and height, displayed if corresponding option is selected in combobox.

Take it in account when effectively creation the blank doc.
