Allow additional inperson/online workshops for each membership

We used to have a field for "additional workshops" in the membership view; I'm not sure what happened to that.

We should now have a field for "additional inperson workshops" and "additional online workshops" in the membership model/view. This will be used when a member site purchased additional workshops beyond what was allowed in the original member agreement.

This will be used in the declining balance view (#2389), as the total number allowed will increase.
