Add Azure Identity login

There are two ways of authenticating to Azure OpenAI Services. As of today CodeGPT supports using the Active Directory key, but users have to manually refresh the token themselves.

This is a feature request for adding the proper way of:

• authenticating to Azure AD
• refreshing the token
