Contributor agreement form demands "Canonical Project Manager or contact"

This was reported on IRC in #ubuntu-motu:

I sent a PR to adsys repository on GitHub and they were requiring me to sign the CLA. I filled out the form but they are asking me to add the Canonical Project Manager or contact. I don't know what it is and where to find.

Indeed contributors are being redirected to https://ubuntu.com/legal/contributors/agreement, and if you select "I am signing as an individual contributor" it requires that field. This doesn't make sense given the usual contributor flow which is from GitHub repo to that page. They can't reasonably be expected to know the answer to that question. This adds unreasonable friction to the CLA signing process.

Please could you fix the wording and only ask for information that they can be expected to know?

Closing, as this has been fixed in the recent overhaul of our CLA
