Various kill party bugs from Killing Cave

For the record, "Killing Cave" is the name of a test-case scenario created by ADoS. In fact, as far as I'm aware it was made specifically to test the behaviour of the "kill" special node, which this issue is about.
I made that scenario. It is more than just test-cases. I tried to make it funny. But it was, indeed, to test different circumstances in which the PCs could be killed and then be raised. There are five, exploring death in and out of combat, in and out of town, and in dialogue with an NPC, IIRC.
More specifically, the demon will kill you in dialogue. The demonic statue kills you in town, in combat. The lavafall kills you in town, out of combat. The frost giant kills you outdoors, out of combat. The undead in the cemetery kill you in combat, outdoors. In all five cases, all of your PCs die, and then are raised with a Kill/Raise Dead node.
