Frontend missing team path suffix on new team creation screen

Issue Summary

Teams are assigned URLs, formatted {app_domain}/team/{slug}.

The team creation page visually omits the team/ portion, making users believe their unique slug will be attached directly to the app domain root.

Steps to Reproduce

1. Visit new team creation screen
2. Observe missing team/ on the path

3. Observe profile page of newly created team (additionally, observe the extraneous app. prefix here - too minor for a second issue)
