Annotation in videos

Is your feature request related to a problem? Please describe. I'm always frustrated when trying to annotate a video. (or even general documents, but videos are really irritating.)

Describe the solution you'd like Any of these would work, I have added stars to indicate preference, more stars are better:

1. Adding the title of the video to the downloaded document, so I can highlight + comment on it.
2. Allow users to comment on area highlights, optionally have the text of the comment appear in the area highlight. Or create a new thing which does this (sort-of a digital sticky note.) *
3. The ability to add comments into the annotations view independent of the having anything highlighted. *
4. The ability to add text directly to the document, this could also take the form of 2. **
