Use BT_DEBUG to define btAssert

  1. Rely on BT_DEBUG to define btAssert, since BT_DEBUG is already defined if DEBUG or _DEBUG are defined
  2. Avoid errors if BT_DEBUG is already defined (e.g. defined by the client)
