Escape HTML entities in failure messages, not just the body

https://github.com/buildkite-plugins/junit-annotate-buildkite-plugin/pull/116 fixed HTML entities being embedded in the failure body, but not anywhere else.

https://github.com/radiopaedia/junit-annotate-buildkite-plugin/ has fixes for this and some other mild annoyances I encountered; feel free to upstream them.
