Exercise names within programs do not update to match edited exercises

Describe your issue Hey,

Whenever an exercise is added to a program/routine, any further changes to the exercise's name are not reflected within the workout program. You would need to remove the exercise from the workout program and add it again.

To Reproduce Steps to reproduce the behavior:

1. Go to 'Exercises'
2. Click on 'Add new Exercise'
3. Set Exercise Type to any
4. Set Exercise Name to "Change Me'
5. Go to Programs List
6. Create New Program titled "Test"
7. Go to editor
8. Add any number of sets of Change Me exercise
9. Go back to exercises
10. Rename "Change Me" to "Name Changed"
11. Go back to Programs List
12. Go to "Test" -> Editor
13. Notice that the exercise name is still "Change Me"

Expected behavior The exercise names within existing programs should automatically update to match any changes to the name of the exercise
