Added a title parameter and a bookmarklet

I added an optional title parameter to the index.php, and I added a bookmarklet to the README.md. You can now click the bookmarklet to immediately display the current page you're browsing into your own installation of ish. The title of the page is passed on to ish, which is handy for presentations where you have multiple ishes in multiple tabs.
