Corrected APA Format (Fixed in Attached File)

Hi there,

I noticed that the formatting of APA references wasn't quite right (a double period following the authors, a lack of necessary italics on the journal volume number, and strange commas within the edition / volume / pages in parentheses for book chapters), and I've updated the template file to address these issues (can be seen live here).

I'm just a hobbyist when it comes to coding, so I'm not sure how to properly submit these on Github, but I've attached the fixed .tpl file in case you'd like to use it in a future version of the plugin.
