Am I doing it wrong?

Hey,

So I downloaded the .zip and uploaded to WordPress via Add New >> Upload Plugin and then activated it. I can't get it to work to save my life. I have col-lg-4 and col-lg-6 as custom classes for some fields and they still generate "-wrap" at the end when I preview/post a form. Am I missing any steps? Help is much appreciated.
