The plugin does not initialize default forum notification settings

Upon activation, default forum notification settings are not applied to existing users. Upon new user registration, they are not applied either.

Here is a proposed simple fix for this issue:

If you can include it in the next plugin release, this would be great. This fix is in production on the sites of my clients.
