Combine the LEO DOCUMENTS and LEO OUTLINE or LEO FIND panels

Make leo documents (with leo icon) be root nodes of leo outlines into a single panel instead of two separate panels.

This will enable seeing more than one outline structures at once.

Also, Outline Node ids (and 'gnx' ids of bodies) need the 'opened document' id concatened in their ids to distinguish them in case of gnx duplicates from an outline to another.
