After playing for some time, the frame rates feel low
After playing for about 10-15min, the game starts to feel like im playing at 30fps, as soon as I restart the game the game feels smooth again. Maybe a cache issue?
I don't have this problem here. I actually do have the same problem.
You may want to be more specific about your environment (OS, graphics card, etc.). Also, is the Diablo 2 game itself vanilla or modified? If modified, could you test with vanilla and see whether the problem persist there?

On fresh startup performance is flawless after 20+ minutes of farming (Pits) I'm noticing huge frame drops.
I'm not playing vanilla - Plugy + Slash BH 1.9.9 loot filter which I suspect might be the culprit. WIndows10, GTX970
Same here. Happens after some effect heavy farming for me. I spam frozen orbs with a "Voice of Reason" weapon with a zealot. Frame counter still shows 155fps but it feels like motion prediction turns off and the game is runing with 25fps again internally. Restart fixes this.
