Support refactor/extract to variable

Describe the bug When selecting code and right-click, I no longer see options to "refactor" or "extract" (to variable/function). I'm very sorry if this is not related to this extension. I sent an issue to vscode, and they say it's related to an extension, but I cannot find which one. https://github.com/microsoft/vscode/issues/121128

To Reproduce Just right-click on a piece of php code, and look at the options.

Expected behavior I'd like to be able to refactor, or extract to a variable/function, the piece of selected code.

Platform and version Windows 10 Intelephense 1.6.3 vscode 1.55.1

Yes, I just purchased the license and found out refactoring is not supported. Very disappointed but appreciate all your work. Hope that you add this soon.
