Add support for Extended Protection

As some web servers are configured to require Extended Protection to allow authentication, it would be preferable that the plugin supports it.

This actually comes "for free" when using Single-Sign-On (from #73). When that feature is finished it won't be a big task to add this also for non SSO hosts.

There seems to be no demand for this feature so there are currently no plans to implement it.
