Bug: X went wrong when using Y (REPLACE THIS)

When I log in with eqMac as a startup app, there is often no sound output at all. I have to click on the Apple Sound Menu (and see eqMac highlighted) and then clicking on HEADPHONES then auto-skips to select eqMac, then sound appears.
