jQuery 2.* Syntax and Outer Width Fix

This contains the fix from #120 but also JSHint / JSLint fixes, and updates everything to use a consistent version of jQuery, where the demos were using 1.9 but the tests were using an ancient 1.4.

On the test.html page in the project root, the header and footer text is shifting to the right when fixed. I need to correct that before I merge this in. I will look into it as soon as I get a chance.

This is such great work! Thank you! I have taken another look, but don't have the time to debug the shift. I will come back to it again soon.
