Errata: Sample code contains a typo (ch.5, pg.191)

On page 191, there's a sample code for a kernel called 'smooth'. The 3rd row of 
the kernel refers to variable 'id' as a buffer, instead of variable 'io'.

The same typo also continues in the explenation which appears after the code 
sample.
