tests: refactor MockUrlFetcher to use files

This is a good idea. I did this for the NPM information but in the interest of getting the tests fixed quickly I reverted to the if-statement approach.

I like `Dokta`. It seems to be in use in the musical world but not in software.

Add ability to detect programming languages

I think https://github.com/dachev/node-cld is for detecting actual languages not programming languages.

GDoc: encoding does not handle `CodeFragment`
