Non-punitive withdrawal form

As a Student I want to be able to submit Non-punitive withdrawal (NPWTHD) from study period form for Ministry review and action.

Acceptance Criteria

NPWTHD Form and Submission

• [ ] Add NPWTHD form under Forms drop down list. Form name is "Non-punitive Withdrawal from Study Period"
• [ ] Pre-populate and display all completed student applications in the drop-down list. Allow student to choose only one application number.
• [ ] Form Content:

Complete all question(s) below Your request will be reviewed by StudentAid BC after you submit the form. Non-punitive withdrawal from study period The non-punitive change to the study period end date can only be made if any of the following conditions are met, otherwise the withdrawal is not qualified for non-punitive change. Please indicate any of the following circumstances that apply to you.* (Radiobutton) I withdrawn from studies due to a post-secondary institution closure (Radiobutton) I did not attend classes at all and repaid the disbursement in full within 30 days (Radiobutton) I attended classes for 10 days or less and repaid the disbursement in full within 30 days (Radiobutton) I completed my first term-semester, including exams (if applicable) and either did not return to full-time studies or attended 10 days or less in the next term/semester. The second disbursement was not issued or the disbursement was repaid in full within 30 days. (Radiobutton) None of the above

• [ ] If student chooses "None of the above", display message "Your applications is not eligible for non-punitive withdrawal change to the study period" and do not allow form submission.
• [ ] Display error Toast message if form submission failed or fields are missing
• [ ] Display an error message if student fails to select a Radio button
• [ ] Display Toast message once Form successfully submitted.

If form has been approved by the Ministry User:

• [ ] Display green "Non-punitive" chip on Scholastic Standing Change in Completed Changes
• [ ] Display a new Green tracker card above Withdrawal tracker card on Student Application:

The non-punitive change to the study period request has been approved by StudentAid BC StudentAid BC has reviewed and approved your request for a non-punitive withdrawal from the study period. The withdrawal has been converted into non-punitive withdrawal and and any associated overawards have been adjusted accordingly. For further clarification or assistance, please contact StudentAid BC.

If form has been declined by the Ministry User:

• [ ] Display a new Red tracker card above Withdrawal tracker card on Student Application:

The non-punitive change to the study period has been declined by StudentAid BC StudentAid BC has reviewed your request for a non‑punitive withdrawal from your study period. After careful consideration, the request has been declined as the eligibility requirements were not met. For further clarification or assistance, please contact StudentAid BC.

Additional Context: Consulted with Service Delivery about removing "upload supporting documnetation" section. Received confirmation that they prefer not have any documents uploaded, as they perform check in Galaxy and do not rely on student's documents
