Student Forms: Ministry Views and Actions the form (part 2)

As a Ministry User I would like to have the ability to review forms submitted by student, action them, and be able to review all the historical data for all forms for each student.

Acceptance Criteria

Forms Queue

• [ ] On the Ministry Portal, create "Forms" queue under Appeals.
• [ ] Once Ministry user clicks on "Forms", proceed to Forms Page. Forms Page should mimic Appeals Page.

Ministry Views and Actions Form

• [ ] Once Ministry User clicks "View" button, open the Form submitted by student. At the bottom of the form display Decision Section. Instead of "Approve for reassessment?" display "Approve?"
• [ ] Save Notes in Notes Section of Student Profile under Forms.
• [ ] If Ministry declines or approves the form, save status as declined or approved, decision to completed and send student a generic email. Update the status in Forms History on Student Portal. GC Notify Template ID: 208b9005-8620-4d2b-90d4-a3ff3303067b

Ministry Views Forms History

• [ ] On Student Profile Create "Forms" Tab after Notes
• [ ] Display Forms History the same way it is displayed on Student Profile
