Student Forms: Tab, Submission and History Page (part 1)

As a Student I want to be able to complete SABC forms online So That Ministry Users can review submitted form and make a decision on next steps. Scope of the ticket: Student Form Tab, Submission and Form History.

Acceptance Criteria

Form Tab in Student Account

• [ ] Create Forms Tab on Student portal, beside Appeals

Submission

• [ ] When student clicks on "Forms", display the Form Submission Page. Follow the Appeals Page Logic and display two Tabs: Form Submission and Form History.
• [ ] Replace "Appeal(s)" with "Form(s)" across the Page. Display the following text:

Submission The forms are used to determine eligibility for a variety of StudentAid BC funding programs for post-secondary students seeking financial assistance and to request non-punitive change to the study period.

• [ ] Display drop-down list where student will be able to choose the form.
• [ ] Student should submit one form at a time.
• [ ] Student does not need to wait for a form to be processed (completed) in order to submit a new distinct form. Similar to "other appeals" - only one pending form per form type .
• [ ] The exact execution logic for each form will be provided in a separate ticket for each form.

Displaying in Production

• [ ] Toggle off Forms in Production until the first Form (Non-punitive withdrwal) is built
