Epic: Student Forms

As a student, I need to have ability to submit forms that can potentially impact my application, account or funding. As a Ministry User I need to have a way to track student form submissions history and outcomes.

Forms to be build on Student Portal:

• Non-punitive withdrawal (MVP)
• B.C. Loan forgiveness
• Peripheral programs (Youth Educational Assistance Fund Transition Plan for Former Youth in Care, Provincial Tuition Waver Program and Learning for Future Grant, Learning for Future Grant)
• Peripheral programs (disability (?) - requires review)
• Appendix 8 (??? - requires review, as also must be signed by Accessibility Coordinator and Medical Assessor)

Student:

• Ability to submit form
• Ability to see the decision

Ministry:

• Ability to view forms (separate queue?)
• Ability to approve/decline form -Action when form is approved/declined
• Automatic message to student with decision and/or Tracker card
• Ability to view historical data for each student (separate tab on student Profile?)
