Student can "View request" for scholastic standing changes

As a student I want to be able see the scholastic standing changes using the request form. This is important as our tracker cards currently indicate that they can do this. Also without this there is no way for a student to know there has been a scholastic change to their account.

Acceptance Criteria

[ ] Have the request form "view request" link appear for students similar to how it does for ministry users (See image 1st image below for ministry view)
[ ] Modify the content for what is shown to the student compare to the form the ministry user sees (See second image for reference) #1, 2, 3, 5, 6, and 7 should appear to students. We want to hide or Disable #4 so that it is not viewable by a student
