Adding question 18/19 from the current part-time application to SIMS.

Acceptance Criteria To the personal information page add the following questions:

• [ ] "Have you ever declared bankruptcy that included student financial assistance?"
• [ ] Add radio buttons "Yes" and "No"
• [ ] "Are you currently in default of a Canada Student Loan (full-time or part-time?"
• [ ] Add radio buttons "Yes" and "No"
• [ ] (To be confirmed) If a "yes" response to either question an E2 restriction should be applied to the student account on app submission
