As a Ministry user I want the Applications page, under Institution requests, updated to use language that better identifies the type of work.

Acceptance criteria:

• [ ] On the left hand menu bar revise "Applications" to "Change Requests"
• [ ] Revise the header "Applications" to "Change Requests"
• [ ] Revise "Requested application changes" to "Pending change requests"
• [ ] Revise "Make a determination....." to "Change requests that require ministry review."
• [ ] In the results section split the 'name' column into "Given name" and Last name" and display results accordingly.
• [ ] Revise "Application #" to "Application number"
