Re-Order Name Fields and Address Fields on Student Profile

Story Description: As a student who logs in with BCeID, I want to input my information in a sequence which is 1) intuitive, and 2) consistent with the sequencing of other areas of the SIMS application, so that I don't input information in the wrong field accidentally and get locked into incorrect information.
