Expected Behaviour Dropdowns should display the appropriate dropdown symbol and the asterisk should display next to the question rather than in the dropdown

Actual Behaviour Dropdown shows up without the appropriate dropdown arrow symbol and the asterisk shows up in the dropdown rather than beside the question

** Steps To Reproduce**

1. Login as a Institution user and click on any Location and select Program. 2) Then click on Create Program
2. The form dropdowns show up without the appropriate dropdown arrow symbol and the asterisk shows up in the dropdown rather than beside the question
