Wrong restriction reason when category is changed

While authenticated as a Ministry user and adding a new restriction to the student account, depending on how the Category and Reason are selected, the Reason can end up with an inconsistent value displayed. As shown below, the number 10 is shown instead of the description.

How to reproduce

• Navigate to the Ministry portal, search for a student;
• On the restrictions tab click to "Add restriction";
• Select a category;
• Select a reason;
• Change the category.

Expected result: while selecting the category the reason must be reset.
