oauth2 support

Bunch of changes to support oauth2. Opening up for discussion.

  • upgrade swagger-ui core code from 2.1.2 to 2.1.3
  • fixed all images assets to load
  • additional configuration variables to control index page
  • o2c template never rendered. added it as the default callback
  • supports forcing ssl
