Normalize script parameters between all types

I noticed that the light-slider was missing `--text` and `--icon` so you have no idea what your slider is for.

Set the name to the entity.name instead of state value

This would make it much easier out of the box and save you from escaping spaced entity names. The toggle already shows on/off state so this is even more useful!
