Refactor npc_trainer

We should port https://github.com/TrinityCore/TrinityCore/commit/bf3ab6d9c4366dae1d34957813e3fbf40ac8532b https://github.com/trinitycore/trinitycore/commit/74d51b9263 https://github.com/TrinityCore/TrinityCore/commit/baebb2d602ca2f10159ea0039ec3a6d03155c3bd to get a more complete and easier to use trainer columns in the DB

It will also reduce the size of creature_template, which is nice
