A workaround is to send SIGCONT in a loop from some shell script, but I'd like to understand and fix it.

That's really a problem in output_stream::write(), it should support iterator+len and iterator pair.

Experimentation can be done with fio, it's just playing with configuration files.

I, too, would like a new release.

It's relevant, but not so important.
