Insert wrapped quotes

When filtering a document, typogruby may convert quotes to entities, but it fails to wrap them in <span class="dquo">.

span.dquo is used for wrapping leading quotes so you can hang punctuation. It shouldn't wrap internal quote entities. Are you seeing this bug with leading quotes or internal quotes?

Typogruby gives us:

“Foo bar”

While I would expect:

<span class="dquo">&ldquo;</span>Foo bar&rdquo;
