CSV required layout

What should the layout of the csv be exactly, if we want to import from the .csv? I've tried 1) having attributes in one column and levels in other (with attributes repeating); and 2) having both attributes and levels in a singe column separated by a space. However, both generated an error "could not open file". Thanks!
