Update DCI decksheet

This removes the "Table number" box and text, adds the reminder maindeck and sideboard count texts, and updates the "Maindeck continued" header.

@Nightfirecat The column headers are underlined in the new version, too

For now, I'm inclined to leave those be. I don't think it's too important to underline them. :woman_shrugging:
