Show multiple outputs as one big screen

This would mimic the current x11vnc behavior, and stitch all the currently active outputs into a single giant tapestry over VNC.

This will take some pretty major refactoring of internal data structures and may not be worth the time now that most of the use cases in #112 are solved and documented.

Leaving this as a placeholder to gather feedback and gauge interest.
