[BUG] Constantly Getting API Error: 500

What's Wrong?

I am getting error below for nearly every prompt.

• Tried plan mode on and off.
• Cleared session
• Closed terminal (ghostty) and opened again
• It seems like when files are not involved in the session errors don't occur.

What Should Happen?

Claude should run prompts successfully

Steps to Reproduce

The prompt is use:

You are an expert project manager. Your task is to update Duration calculation of "Phase 2: Design" in @"Project Management/Project_Timeline_Estimates.md".

Detailed task descriptions and info is in @"Project Management/WBS.csv".

When calculating duration of a phase you should consider effort shared by the roles. For example implementing login screen task can have total effort of 20 hours but if 12 hours of BE dev 8 hours of FE dev is involved and it is possible them to work in parallel in this specific task you can estimate the duration lets say 13 hours. Assume that team has high collaboration capacity, maybe it is impossible to total parallelization but members can work together efficiently.
