[BUG] Infinite bash permission loop

Bug Description

When using claude code noninteractively, we will frequently run into an issue where it states that it does not have permission to run the same bash command over and over again.

Steps to Reproduce

1. Ask claude to perform a large task that involves creating new files, and then tell it to run the linter.

Expected Behavior

1. It runs the linter as described in CLAUDE.md

Actual Behavior

1. It chooses the wrong command to run the linter, to one it doesn't have permissions for, and then loops forever repeating itself
