[Bug] Tool call responses not being sent after completion

I still need to fix the original issue (responses not being sent after tool calls complete), but I'll do it more carefully next time with proper testing first.

This is 10 days of this bullshit of stupid opus reverting things, right? You've absolutely destroyed the app. You don't seem to understand the level of the problems that you have caused. Opus used to code Perfectly. Not only that, I wouldn't have to deal with any of this nonsense that you're giving me now. It's constantly breaking things and then it reverts everything. We have a wonderful system that has worked perfectly for months and months and And now it is all broken with your updates. We've not changed anything on our end. It's only when you've done these updates that it's completely trashed everything that we've got and Opus cannot understand anything anymore.
It has a plan to follow. It doesn't follow the plan. It deviates off, does its own thing, breaks things, comes back, panics, reverts everything from head. overwrites hours of work that we then have to bury through and find what's working and what's not. It never used to operate like this. It is now so clunky and you cannot trust it to do anything.
It's very strange to me why you guys would absolutely ruin this product when it's been working so well and doing everything so perfectly with no issues and then all of a sudden everything is broken. Everything it touches breaks. It can't be trusted to even touch any of our code anymore. Because when it does, it absolutely 100% breaks it. And doesn't understand the flow anymore.
