[BUG] Errors out when trying to read an image file

What's Wrong?

When claude code tries to read an image it response with an error and continuing the chat is not possible anymore after that.

What Should Happen?

Well, it should not responde with an error obviously.

Steps to Reproduce

1. Create a folder with an image inside
2. Tell claude to read the image (What do you see in this image: "path to image")
